My job consist of, So i usually do 12 hour shifts. So when i get here i usually do the narcotic count and take report on the residents i will be taking care of for the duration of my shift. I will get vitals and assess everyone of my residents, if we are at capacity that usually means i could be taking care of 48 residents. When i get that done ill start doing my medications to deliver all medications to every resident. I then prepared the medication and verify the patient by asking them there name and date of birth then i verify the medication 3 time to make sure the dose is correct, the medication is correct and i have the correct medication for the correct resident. This includes EYE Drops, Ear drops, insulin and finger sticks, I then administer medication and sign the medications given off and marked the refusals as refused, also if on psychotic medications have to chart on behaviors, side effects if any. Ask each resident about there pain and have them rate it for me from 0-10. once im done given all medications for the 6a medications. I then do each treatment with we also do our own measurements of of every bruise, pressure ulcer, abrasion. Also have fall charting, skilled charting. Chart daily on each resident and there current status. Around 11am i go get finger sticks and administer medications that are due at that time, once i finish med pass i usually put in new orders from nurse practitioner, along with place order for labs and narcotic. I usually make sure the narcotics are ordered in a timely manner. So usually at 2p that what we call shift change but were i do 12 hour shift i usually catch up on charting and then at 230p i start my next med pass. So i then go down and do another medication pass with accu checks and administering insulin, checking medication 3 times verifying that i got the correct resident. i do that for my 48 resident's, if PRNs are asked for i usually administer within a timely manner. I then do my fall charting, skilled charting, behavior charting, chart on my residents for the time i have them, have clear communication with the doctor, family , and my supervisor. I then put medications away from pharmacy drop off. and then at 6pm its time to count the narcotic drawer and give the next nurse a detailed report and the residents they we be taking over for.
