Ensure timely MDS assessment completion and submission-
Performed essential MDS activities (Scheduling, assessment,
batching/transmission, monitor assessment quality by conducting IRR
Served as a resource and liaison for RAI-MDS issues. Provided
training, guidance and assistance for team members to optimize
software (Caribou) for MDS and care documentation.
Assist to trouble shoot problems, create a remedy ticket for unresolved
issues, alerts in Caribou CLC suite.
Review updates and new information impacting RAI/MDS process and
Long-Term Care via CMS website, National Directives/Policies. Assess
and evaluate the quality of care being given to long-term care
residents.
Engaged in Quality monitoring (QM) activities -Present QM report and
propose plan for areas of improvement, reevaluate plan effectiveness
Provided training and guidance on patient centered care planning and
collaboratively devised reevaluation processes
Served as moderator for IDT/ Pre IDT/ neighborhood rounding
meetings communicate core discussion points to leadership as needed
Serve as integral part of unit leadership to ensure care coordination
and compliance, served as a chairperson for fall prevention committee,
represent the unit in evidence-based practice Research committee
(EBPR).
Collaborate with education department to disseminate the latest
evidence base practice and research findings to team members.
Conduct in-services on various topics including data driven resident
care, the need to ensure data integrity and security, how to use data
to improve care and health outcome, how to promote prevention than
reactiveness, the way towards high reliability organization, the need
for transformational leadership, advocacy, need for resident
engagement etc.
Proactively spearheaded communication streamlining effort between
IDT, leadership, resident and their families

Actively participated in survey readiness and presented necessary
documentation for surveyors during surveys
Demonstrated ability to lead by example and work collaboratively to
achieve organizational goals.
Charge nurse -As a charge nurse, assigned staff to activities based
on qualifications and monitor smooth flow of tasks on the unit, resolve
problems between staff members or residents or both in collaboration
with supervisors and management.
