Educated Case Manager dedicated to providing assistance to individuals and families in need. Proficient in assessing individual circumstances, identifying resources and developing plans of action to deliver support. Proven ability to work independently and collaboratively with diverse stakeholders.
• Communicate effectively with patients, promote peer perspective, and facilitate peer support groups.
• Link patients to resources directly or through collaboration with the treatment team.
• Assist with outreach and promoting HIV/AIDS services. Design various types of health education materials for PLWHA.
• Recruit patients for educational sessions and administer questionnaires, data gathering and assists with event organization.
• Identify patients, who have been lost to care, or newly diagnosed with HIV and need navigation in order to get into HIV care. Educate newly diagnosed about risk reduction and medication adherence.
• Educate and teach communities, organizations and individual on Prep,Pep, HIV & AIDS, STI's, Stigma, Adherence etc. through educational presentations.
• Conduct outreach to run away, homeless, and at-risk youth in the community, connecting youth to needed services with an
emphasis on linkage to stable housing.
• Complete intakes and assessments to youth referred to the Graduated Apartment Program and A Step Forward (ASF).
• Take crisis calls that represent a variety of situations.
• Make appropriate referrals to other service providers (substance abuse services, psychological and domestic violence.
• Provide youth with regular support, advocacy, resources, and referrals in accomplishing their goals.
• Utilize solution focused strategies and motivational interviewing techniques to assist client in progressing towards their identified goals.
