This committee reviews falls to find their causes. We use research-based standards to create plans that help prevent falls and improve patient safety.

**Safety, Serenity Hospice**

To enhance care at our Hospice Home, we created a visitor monitoring system. Visitors provide their name and purpose upon entry and sign out when leaving. This helps ensure the safety of patients and staff.

**Fall Prevention Serenity Hospice**

In monthly meetings, we analyzed falls and created preventive plans, including a home checklist for patients and families. We focused on the '3 Ps'-Pain, Potty, and Position-plus hunger to improve safety.

**Fall Prevention, Pinecrest **

We phased out individual fall alarms in the Memory Care Unit by monitoring each patient. We stopped alarms when possible and reactivated them only when necessary to reduce overstimulation.
