-Assess and record changes in patient behavior/clinical stability.
-Perform physical examinations and diagnostic tests.

-Advocate needs to promote wellness of patients
-Collect patient health history.
-Collaborate with medical providers to improve healthcare processes and improve patient outcomes.

-Educate patients and families on treatment plans.
-Administer medication, change wound dressings, advocate patient needs.
-Read and interpret patient information, and making treatment decisions when appropriate.

-Appropriate delegation of care interventions to nursing assistants.
-Document aspects of patient care via Meditech.
-Execute hospital admissions and discharges.

-Assist residents with transferring, ambulating, eating, dressing, bathing, preventing skin breakdown, maintaining personal hygiene, etc.
-Routinely ensure resident needs and preferences are met.
-Answer various calls for help and observe changes in resident behavior.
-Serve as an advocate for residents.
-Serve meals and clean eating areas following mealtimes.

-Facilitate resident visits with family and/or healthcare providers.

-Accurately document aspects of care provided.
-Maintain up-to-date status with trainings and/or inservices.
