· Maintain accurate and detailed medical records, documenting patient care, observations, and interventions under the supervision of the nurse
· Implement pain management strategies, including alternative therapies.
· Provide emotional support to patients and their families during critical and often traumatic experiences.
· Perform thorough assessments of trauma patients.
· Assessed and prioritized patients need to develop personalized care plans
· Take vital signs, monitoring patient conditions, performing basic assessments as directed by the supervising nurse.
· Assist with activities of daily living like bathing, dressing, eating, and toileting.
· Effectively communicating patient status updates to the supervising nurse and other healthcare team members.
· Gathers and delivers medications to/from patient care units
· Fills and prepares Omnicell for patient-specific medication needs
· Prepares intravenous admixtures, including chemotherapeutic agents
· Prepares stock requisitions and places stock in proper storage locations
· Return medications to stock
· Rotates and inspects medications for expiration dates
· Receives medications from the storeroom or outside delivery
· Fill orders for par-stock replenishment, as needed
· Prepares extemporaneous solutions and/or ointments, as needed
· Prepares medications for unit-dose distributions
· Answers and screens telephone calls for pharmacist, as needed
· Troubleshoot problems related to Omnicell stations
· Perform documentation, as instructed
· Perform other duties as assigned
· Provide access to medications for patients by completing Prior Authorization Requests, Co-pay Assistance Programs, Medication Assistance Programs, and benefits investigation.
· Coordinates with third-party insurance, clinical staff, and patients to effectively process prior authorization requests for medications within the prescribed timelines.
· Directly enrolls eligible patients into manufacturer patient assistance and co-pay programs.
· Provide support in medication adherence opportunities as well as other pharmacy related programming, including patient outreach, data/metric tracking, trending, and reporting.
· Interacts with all levels of medical staff to discuss relevant patient information as warranted.
· Conducts chart review and patient interviews to identify patient specific reasons for poor medication adherence; resolves identified problem or escalates to clinical pharmacist.
· Gathers data and generates reports to improve clinical pharmacy quality metrics.
· Initiates and facilitates all drug purchasing for the pharmacy department.
· Monitors stock levels; evaluates previous purchasing and utilization patterns and anticipates needs.
· Maintains appropriate drug inventories.
· Coordinates the receipt of all deliveries, restocks storage areas and rotates stock as necessary.
· Facilitates all returns and credits of expired or unneeded drugs.
· Assists in maintaining the pharmacy technician schedule.
· Assists in the development and training of new pharmacy technicians.
· Works with the pharmacy manager or director to identify areas of improvement in the workflow or inventory control of the pharmacy.
