  • Provide nursing care to patients in the home setting such home infusions, chemotherapy, & patient teachings on how to self-administer medications through central lines (I.E. PICC’s, ports, midlines, tunneled & non-tunneled central catheters)
  • IV medications include antibiotics, inotropic meds, chemotherapy, immune modulators, parental nutrition, normal saline, etc
  • Acts as a preceptor to new nurses coming onboard
  • Patients are taught how to manage their long-term IV catheters & how to trouble shoot the line
  • RN to also trouble shoot line when patients are unable to solve the problem on their own
  • RN was responsible to be on-call at least once a week
  • Responsibilities included admitting new patients on service, triaging the patient over the phone if experiencing any adverse drug effects from the medication, trouble shooting line issues over the phone w/ the patient/CG, & as needed dressing changes
  • RN performs disease specific home infusions such as Remicade, Entyvio, IVIG, etc. for patients with chronic diseases. A peripheral IV is started & then removed at the end of the visit
  • Patients are monitored during & post infusions for any adverse drug reactions
  • Provide IV infusions in the Adult Infusion Suite (AIS) as needed when AIS RN is out of the office
  • RN accesses/de-accesses ports for patients that are on chronic therapies such as hydration, TPN/nutrition, chemo, etc
  • RN gives subcutaneous injections such as Crysvita, to patients with chronic illnesses
  • RN performs routine central line catheter care/dressing change and lab draws weekly for patients receiving long-term IV antibiotics
  • RN can administer PRN Alteplase for a central line that has no blood return
  • RN can also collect labs via venipuncture if central line has no blood return pending insurance authorization for Alteplase in the home setting
  • All documentation was through Epic.
