  • Educate referral sources, patients, and families on the benefits of Home Health and Hospice services provided, insurance coverages and qualifying factors
  • Assist patients and families in the transition from hospital/SNF to home
  • Assist case managers/discharge planners with patient discharges i.e
  • Getting required paperwork needed for admission, assisting with DME, IV ABX, wound vac, etc
  • And make sure patient and family are comfortable and understand the discharge process
