Manager of Care Coordination

  • Continued the development of the Case Management Department. The department consisted of two Registered Nurses and one social worker. Currently my department has grown to 3 Registered Nurses, 4 1/2 Fulltime Social Workers, and one Insurance Specialist with solid workflows. Leader index of 100, Tier one Leader 2 years in a running, with high engagement results of 4.93 for Case Management. Two additional departments added since taking over Case Management with engagement results of 4.83.
  • Implemented Clinical High-Risk meeting to monitor, track and evaluate long stay patients as a means of identifying areas for potential practice change.
  • Implemented a Denials Analyst position, to track and trend denials for potential lost revenue.
  • Designed and implemented the insurance specialist position to ensure insurance was correct on the front end to ensure preventative steps are taken, as early as possible, to ensure a much less expensive approach.
  • Implemented a program to bridge the gap between prior authorization at the clinic and the hospital to make less room for error and provide a more cohesive process.
  • Created a track and trend system to follow our peer to peer denials and provide feedback. Help providers decide between accepting observation or appealing inpatient admission.
  • Implemented the two-midnight rule and assisted with updating providers templates. Provided documentation education to physicians if a patient is inpatient, but is not going to stay the extra day.
  • Implemented a coding book (arranged by body systems) to assist physicians and nurses in picking the correct CPT codes to get authorized.
  • Continued the oversight of workflow when changing from Meditech to EPIC. Developed a streamline approach to the workflow from scheduling to pre-op coding, and prior authorization.
  • Developed joint class and assisted with education.
  • Developed a follow up phone call program for patients that had been discharged to meet CMS and quality requirements.
  • Assisted in revising Grand Rounds to help increase effective communication between departments. Communicate a clear plan of care and reduce late dismissals to decrease sub-optimal through put of patient and get beds available sooner for surgical patients.
