Manage all in patient's pre-existing medical conditions and acute care medical , psychiatric or dietary needs during their 28 day treatment stay, Collaborate with Nursing staff and other facility dept, members to meet patient treatment goals .
*Perform in home and/or virtual annual patient health care assessments for major insurance companies.
* Review patient's current PMH, PSH, Family Hx, Social Hx, Psychiatric HX, medications, supplements and active/past treatments,
*Access home environment for safety.
*Review all health care screenings are up to date.
*Identify gaps in health care that can possibly create unnecessary illness, injury or hospitalizations.
*Create patient individual care plans based on all findings specific to each patient.
*Improve patient outcomes by developing and implementing individualized patient centered care plans.

Responsibilities in an Orthopedic outpatient office setting included:
*Assessment, diagnosis and treatment of patient's in Orthopedic outpatient care setting for both acute and post operative patient care needs.
*Performed cast removals, suture/staple removals, Joint injections, joint fluid needle aspirations, wound care, splint /brace support placements and removals
* Managed patient pain as needed for acute and chronic Orthopedic conditions
*Order and interpret diagnostic tests to develop and implement treatment plans.
* Prescribed appropriate pharmacologic and non-pharmacologic treatment modalities to support health and comfort of patients.
* Advised caregivers and family members in providing emotional and psychological care to support patient needs.
