Responsible for implementation of the nursing process in all aspects of patient care for all patient populations including geropsych, chemically dependent adults, adult psych, adolescents and children. Performing comprehensive assessment of patients' nursing care needs including physical, behavioral, psychiatric, and social needs. Observing and analyzes patient behavior and status, and documents in the medical record or reports, as needed, to other team members, including the attending psychiatrist. Interacting therapeutically with the patients and assisting in maintaining safety conditions. Collaborating closely with other coworkers, Nurse Practitioners and Therapists to provide a therapeutic milieu. Using training skills consistently to redirect patients. Monitoring patients consistently for their security and safety.

Developing or contributes to the development of the individualized recovery plan in collaboration with other recovery team members, based on data obtained from the assessment. Providing comprehensive individual or group health teaching and coping strategies to patients and/or families in order to meet patient/family needs.
Utilizing one-to-one counseling skills in therapeutic interactions with patients to assist them in identifying anxiety and developing coping strategies. Initiating and implementing population-specific nursing interventions based on appropriate standards of care for the assigned patient population. Administering medications and treatments as ordered by the physician and maintains medication administration (MAR) and controlled drug records according to facility policy. Establishing and monitors the therapeutic milieu assuring a safe and therapeutic environment for the patients. Providing training to other staff in psychiatric nursing principles, unit routines, and milieu management. Initiating and monitors the appropriate use of restraint or seclusion to ensure the safety of the patient and staff members.

Provide case management work. Develop and maintain long-term contact with clients, client families, and service providers for medical, social, educational, and related service needs. Provide rehabilitation skills training for assigned consumers. Develop rehabilitation treatment plan and development and delivery of rehabilitation services and documentation of progress. Help consumers with the development of community living skills, increased management of mental health symptoms (symptom management), increased knowledge about medication, supported employment skills training, decreased psychiatric hospitalizations, and increased community tenure.

Conducting screening and risk assessment interventions per program guidelines. Provide outreach and preventative care to members seeking services in the least restrictive environment. Identify special needs members through the completion of health screens and other resources. Educating providers, members and their families on behavioral health issues including symptoms, relapse prevention, stress reduction, lab work, and healthy lifestyle choices. Coordinating behavioral health treatment, and discharge plans, with Primary Care Professionals (PCPs) and behavioral health providers using a script of structured questions. Participating in the development and modification of care plans in conjunction with member, members families and managing physician to include, monitoring for treatment compliance and assisting with problem solving in breaking down barriers to treatment to include referrals to community resources. Assisting members with locating providers and scheduling appointments. Assisting with development of plan specific literature and education materials in conjunction with medical director and corporate oversight. Coordinating urgent care evaluations when necessary.

Coordinating and providing direct psychosocial services for consumers with psychiatric diagnoses and chemical dependency. Performing Crisis services (as needed) in order to assist in stabilizing and reducing symptoms of mental illness or emotional disturbance. Completing assessments i.e. ANSA and developing individualized treatment plans for consumers. Ensuring clinical documentation is submitted in accordance with Center standards and meets all designated requirements. Linking and referring consumers presenting to clinic to appropriate services (internal and/or external) as indicated i.e. Community based programs, state programs, public housing, temporary housing. Conducting consistent visits to consumers in a variety of settings to ensure health and well-being and completion of treatment goals. Utilizing outcome evaluations to improve treatment services. Educating consumers to promote and develop independent living skills, effective behavior management, and proper use of medication.

Providing on-going management services for adults with mental illness under the direct supervision of the Mental Health Director. Responsible for direct care to consumers in the clinic and in-vivo as required by policy and contracts. Adhering to recovery model and striving for the successful discharge of each customer served. Responsible for coordination, planning and implementation of treatment plans. Attending meetings and training as directed by supervisor. Managed caseload effectively, prioritizing tasks according to urgency while maintaining high-quality service delivery. Ensured compliance with documentation standards, completing accurate and timely progress notes for each client interaction.
