  • Maintained database of housing resources, landlords and management companies. Collaboratively developed and implement effective housing navigation plan to cultivate pathways to short/long term sustainable housing with people served.
  • Negotiated with landlords and real estate agents to obtain vacant apartments for clients with histories of homelessness, mental illness and substance abuse, occupying apartments by providing direct, hands-on support services.
  • Supervise staff members who have clients, and monitoring case loads, writing weekly case notes on home visits, and services provided to the clients.
  • Approached issues proactively to best meet current and future community needs. Identify , coordinate and support appropriate services (e.g. DTA, SSI, Rehab, etc) in serving the clients.
