The HIV/STI Prevention Team Lead supervises the activities of the Bilingual HIV/STI Prevention Specialist and the PrEP Navigator to elevate services supporting the Entre Hermanos Prevention Program targeting Latino MSM, to prevent the spread of HIV/AIDS.

This position is responsible for managing quality sexual health related counseling and HIV/STI testing in an engrossing environment, providing test results and education around safer sex, assisting with community outreach strategies This position requires one-on-one contact with individuals in Yakima County.


SPECIFIC DUTIES:


Collaborate with the PrEP Navigator to achieve the goals of the program.


Collaborate with the HIV/STI Prevention Specialists to run testing services.


Recruit Latinx LGBTQ+ members for scientific projects.


Develop reports, follow guidelines, and implement testing policies

Handle administrative matters and ensure that all program/project activities are implemented in accordance with established policies and procedures as well as grant requirements.


Collaborates with Washington State Department of Health (DOH) to improve targeted HIV/STI testing.


Collaborates with EH PrEP Navigator to increase the availability, accessibility, and utilization of pre-exposure prophylaxis (PrEP) to high-risk individuals.


Follow policies and procedures on individual patient management, laboratory procedures, counseling techniques, public health statutes, and rules/regulations on reporting STIs.


Assess the level of health risk related to HIV/STDs in each client and determine appropriate screenings and/or referrals done by the HIV specialist.


Supervise the delivery of confidential test results by STI Specialist to clients and link positive test results to treatment and care for a variety of STIs; complete requires HIV/STI case reports.


Tracks and documents in a DOH-approved data system the outcome of referrals.

The Non-Medical Case Manager plays a vital role within the interdisciplinary Ryan White Part A.Providing culturally responsive, client-centered support to people living with HIV. This position focuses on improving retention in care by addressing key social determinants of health, including consistent access to HIV-specialized medical care, treatment adherence, and stability in essential resources such as food, housing, and transportation.


Working under general supervision and in collaboration with program staff, the Non-Medical Case Manager ensures client eligibility for core medical and supportive services, assists with documentation and referrals, and connects clients to social, legal, and community-based resources. The role combines compassionate client engagement with administrative precision to promote equitable health outcomes and support the overall wellness and dignity of the Latino/a/x LGBTQIA+ community



DUTIES AND RESPONSIBILITIES:


· Coordinates client documentation to maintain eligibility for a variety of core medical and social support services for case management programs.

· Conducts client interviews required to complete intake applications and all required forms for core medical and social support services.


· Works closely with medical case managers and HIV peer navigator to reduce client barriers to obtaining needed medical care and /or social services.


· Assist with the identification of community resources that build support for medically and socially complex clients.


· Maintain current information on all regularly accessed community resources for Case Management.


· Under the supervision and guidance of the RWPA Program Manager, co-develops, coordinates, and maintains.
