• LPN/LVN role is responsibility of assisting patients and performing or assisting with all clinical services available at the clinics. The LPN/LVN/RN will perform patient registration and intake tasks, will assist with wait room management and other tasks to enhance the patient experience. • Under the supervision of a Nurse Practitioner, Assist with greeting patients, taking vitals, complete accurate patient documentation via computerized charting, as well as duties such as supply ordering, inventory and weekly patient logs.
• Conduct all activities related to screening patients for COVID19: patient registration in EPIC EMR, insurance verification, point of care test specimen collection/processing, notifying patients of results, educate patients on appropriate next steps based on test results. • Provide professional nursing services under the direction of a RN in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirement, applicable healthcare standards, governmental laws and regulations, and Gentiva Health Services policies and procedures.
• Provide skilled nursing interventions in the treatment of the patient/clients illness, rehabilitative needs and preventative care. Utilize a holistic approach in the provision of problem specific therapeutic interventions, teaching and training activities in accordance with the plan of care.
• Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
• Confer with the RN Supervisor regarding needed changes in the Plan of Treatment. Accept verbal orders from physician were permitted by state law/regulations/Nurse Practice Act and communicate these orders to the RN Supervisor.
• Utilize a systematic, individualized goal/outcome driven approach in implementing the nursing plan of care.
• Maintain contact with patients, physicians, clinical manager(s), other members of the healthcare team in a timely manner regarding patient/family needs and status changes. Participate in care coordination activities and discharge planning as appropriate.
• Maintain the highest standards of professional conduct in relation to information that is confidential in nature. Share information only when the recipient's right to access is clearly established and the sharing of such information is dearly in the best interests of the patient.
• Appropriately communicate to ensure adherence to professional standards in the provision of and availability of supplies, materials and equipment needed to safely and effectively implement the plan of care.

• Providing high-quality in-home care to pediatric patients that require 24/7 nursing care. Provide (daily) paperwork as required by agency to maintain accurate and complete medical records. Provide nursing care in accordance with the written plan of treatment signed by the physician and all subsequent orders that authorize and direct changes. Work 1:1 building a relationship with each of the patients with a commitment to service and quality that shows compassion and positive patient outcomes. • My job consisted of greeting patients at their appointment times, take vital signs, escort patients to rooms, data entry, answering phone calls, schedule referrals for patients, use of electronic health records and standard office programs. Take patients health history and reasons they are seeing Dr. Administration of immunizations, allergy shots, and any medications per Dr. orders. At the end of the day part of my job was to restock all the rooms and make sure any and all supplies were not running low.
