• Work as a liaison between doctors, patients, coordination of care and therapy to ensure individualized care plan goals are known and being effectively worked towards upon patients discharge
• Perform complete patient assessments, both physical and psychological to ensure patient is receiving most effective and superior care possible
• Monitor and provide care for all surgical sites, dressings and drains
• Observe patients for changes and implement nursing interventions to respond promptly to all medical issues
• Provide comprehensive instructions on discharge plans to sustain continuity of care upon discharge
