40 Hours Per Week - Full-Time

• Triage walk-in patients and manage urgent care needs using the nursing process and clinical judgment.
  Conduct screenings for depression, suicide risk (C-SSRS), alcohol use, and chronic disease management.
• Educate patients via in-person, phone, and VVC appointments on prevention, wellness, and symptom awareness.
• Collaborate with providers and the interdisciplinary team to enhance care coordination and improve PACT metrics.
• Performed foot checks for diabetic patients, and provided targeted education.
• Administer vaccines, perform EKGs, check blood sugar, and deliver IM injections.
• Assisted diabetes educator with educating veterans with their continuous monitoring device.
• Acted as the point of contact for the PACT team, and assisted colleagues by helping with scheduled appointments, creating nurse assignments for the day, patient education, and computer malfunctions.
• Created a nurse check-off sheet for the end of the day to ensure organization and that tasks were completed by the nursing staff.

40 Hours Per Week - Full- Time

• Delivered compassionate end-of-life care, and emotional support to patients and families.
• Unit champion for dialysis and glucometer audits and compliance.
• Communicated with interdisciplinary teams to address patient needs and care plan adjustments.
• Admitted new veterans and conducted skin, pain, cognitive, and fall risk assessments.
• Presented evidence-based practice articles as part of professional development.
• Maintained professional boundaries and followed the ANA Code of Ethics and HIPAA guidelines.

Part-Time

• Provided basic patient care, including hygiene, mobility, and wound care.
• I shadowed licensed nurses and assisted with IV insertions and medication administration.
• Cleansed and changed ostomy bags, following proper infection control procedures.
• Presented the assignment for the interdisciplinary team during the morning huddle.
