SSVF Street Outreach Specialist

As a member of the SSVF Outreach Team, I worked to ensure any unsheltered Veteran in Salt Lake County is engaged & has access to housing resources. The goal of our team is to provide low-income Veterans with a variety of supportive services and to promote housing stability.

  • Provide direct case management, including crisis intervention, assessments, goal setting, problem solving, agency and community resources utilization, and network building.
  • Working with our diverse guests on your set caseload to set housing goals
  • Engage with all guests during daily open office hours to connect them with resources
  • Setting healthy Boundaries with guests

  • Delegate tasks in a fair manner to ensure needs of the facility are met. This includes Incident Reports; rounds & tracking; daily counts; intake support and other duties as necessary.
  • Ensure all daily tasks required of the Family Advocate Team are completed efficiently & effectively.
  • Serve as the point-of-contact to facilitate and/or assist with any/all emergencies, disaster, evacuation and safety plans. Physical presence is required during all assigned shifts.
  • Provide preliminary staff scheduling and on-going support for any/all shift change needs.
  • Provide direct assistance and information to Family Shelter residents after initial intake and throughout their stay in a caring, compassionate, trauma informed manner.
  • Communicate the needs of staff and clients to the Family Advocate Supervisor and Director of Family Crisis Services.
  • Complete shift reports for oncoming shifts, ensuring all pertinent information is relayed; including incidents, police/EMT calls, staffing situations, etc.
  • Provide residents with understanding of all safety-based Shelter rules, and ensure residents are adhering to Shelter rules and policies.
  • Conduct resident bunk searches to ensure no contraband is within the Shelter.
  • Track and compile monthly Incident Reports & Exit Logs, and create end of month spreadsheets.
  • Create Incident Reports when necessary, and document them in the appropriate logs.
  • Identify and report problems or conditions to proper departments.
  • Communicate with Supervisors, Coworkers, other departments and community partners using e-mail or in person contact.
  • Assist in educating Staff members and Shelter residents about available resources.
  • Crisis Intervention and emergency interface with agencies such as: Paramedics, Fire Dept., Police Dept., Adult Probation and Parole, and Child Protective Services.
  • Responsible for the Motel placement of current Residents. Conduct weekly assessments of housing progress to ensure continuation of Motel stay.
