Provide MNT (Medical Nutrition Therapy) for patients with Diabetes including Type 1 DM, Type 2 DM, and GDM (Gestational Diabetes)

• Provide DSME (Diabetes Self-Management Education) for patients with Diabetes – education topics include (AADE 7 Healthy Behaviors): Healthy Eating, Being Active, BG Monitoring, Taking Medication and how to take medication, Problem Solving, Reducing Risks, Healthy Coping
• Works closely with multi-disciplinary team (MDs, NPs, RNs, LPNs) to care for patients with all types of Diabetes
• Reviews patient's blood sugar logs in between appointments and adjust therapy as indicated – medications and diet
• Serves on multi-disciplinary team (NP, MDs, RNs, RDs, LPNs) for Vanderbilt Eskind Adult Diabetes DSME program which is accredited through ADA
• Served as team lead for DSME team leading weekly huddles and monthly DSME meetings with multi-disciplinary team
