• Treated wounds and put in appropriate pressure reduction orders as well as nutrition consult if needed.
• Recommend treatment for wounds.
• Document on wounds/ostomies, communications, patient history and treatment recommendations
• Change wound vac dressings
• Educate patients about cause of wounds and how to manage it.
• Educate patients about new ostomies
• Treat peristomal skin irritation
• Change ostomy pouches

• Admit, recertify and discharge patients

• Educate patients about managing chronic illnesses, medications, fall prevention and community resources
