Work includes providing comprehensive outpatient dialysis care to ESRD/AKI patients on dialysis through weekly rounds, assessing patient’s fluid status, Blood pressures, Lab results, vascular access and overall patient’s clinical presentation. Provides intervention, prescribing medications and giving education regarding out-of-range BP level and fluid status, abnormal lab values including electrolytes derangement, anemia, and Bone mineral abnormalities. Also give patient education regarding diet and fluid management. Assess patient’s vascular access and refer pt accordingly for angiography or if needing to see a vascular surgeon, as well as to catch and manage any potential access infection or access issues. Diagnosed and managed clinic patients seen in Oxford and Durham Nephrology office. These patients’ population are usually composed of but not limited to CKD stage 2-5, Hypertensive patients, patients with AKI and/or Electrolyte abnormalities such as Hypo/Hypernatremia and Hypo/hyperkalemia.
Work also includes a hospital rotation at Duke Regional Hospital (assigned at least one week per month) to render nephrology services for ESRD or AKI patients needing acute dialysis in a hospital setting. Also manages patients with AKI not on dialysis, patients with volume and/or electrolyte abnormalities.
