-Perform data collection, analysis, and interpretation on
the Key Performance Indicators for the department.
Evaluates existing Person-Centered Care processes to
assess for opportunities to make improvements. Assess agency programs for policy and regulation compliance.
-Create monthly data reports through chart audits for
quality assurance and adherence to OASAS guidelines
in outpatient, inpatient rehabilitation, and residential
programs.
-Facilitate workgroups and LEAN initiatives to engage staff and referral sources in improving substance use treatment programs.
-Lead new projects within the department to expand
substance use treatment and harm-reduction services. Utilizes program implementation and evaluation to increase efficiencies in service line delivery.
-Lead agency in obtaining OASAS LGBTQ Affirming Program Endorsement. Serves as LGBTQ+ Liaison for clients and staff to increase department-wide initiatives for diversity, equity, and inclusion.
-Intervene with clients with high-risk factors for
overdose, suicide, violence, and re-admission. Facilitate
priority access into treatment while maintaining the
client's treatment and recovery goals.
-Create daily, weekly, and monthly reports to monitor program demand and volume. Present data in team meetings to use for improving client care practices and productivity.
-Maintain substance use treatment hotline to offer
immediate evaluations for clients. Oversee and
maintain rotation of staff to answer the hotline to
ensure quality and excellent customer service.
-Manage evaluations to Addiction Therapists to support substance use treatment evaluation and crisis management.
-Provide person-centered care through comprehensive evaluations and diagnosis for clients entering substance use treatment. Implement harm-reduction strategy and safety planning for high-risk client population.
-Complete biopsychosocial assessments for families on behalf of the Attorney for the Child in Erie County Family Court to determine appropriateness of placement for children involved in abuse, neglect, and custody cases.
-Perform home visits for caseload to assess for safety of living environment for all parties involved in the court proceeding.
-Provide written court reports in a timely manner to summarize findings for the Attorney for the Child.
