• Responsible for developing and maintaining relations within Community Health Network to create a smooth transition to IUHHC enteral nutrition services
• Met with all new patients at bedside, qualified Medicare patients for coverage, helped the patient with any insurance-related barriers, followed referrals until discharge, provided bedside educations, communicated discharge plans to IUHHC intake staff, and acted as the point of contact for intake questions
• Acted as a resource to Case Management, provided education to inpatient Case Management, Physicians, and Dietitians, and provided education to outpatient clinics/offices

• Taught patients who were discharging from the hospital how to use their feeding pumps or bolus feeds for enteral nutrition
• Assessed patients' clinical symptoms once their tube feeding regimen has started at home and made suggestions for improved tolerance if needed
• Communicated with the inpatient/outpatient medical teams on how to create the best outcome for each patient
• Responsible for all liaison duties between IU Arnett, Bloomington, Community North & East inpatient hospitals and associated outpatient clinics to ensure a clear transition for patients to IUHHC enteral nutrition services

• Supported production of enteral feedings such as breast milk, formula, and pureed food for an average of 200 patients per day at Riley Hospital for Children
• Provided sanitation, cleaning, and infection control practices to ensure proper enteral formula and breast milk preparation
• Assisted with inventory, supply ordering, preparation, fortification, distribution, and processing of enteral feedings
