Patient advocate, liaison and educator focusing on a holistic approach to improving outcomes and and reducing hospital admissions by utilizing community resources and regional partnerships.
Charged to to fill the void in care and assist transition from acute care hospital environment to home in a Comprehensive Stroke Center. Use of hospital and community resources to improve recovery and ease caregiver burden. Collaboration of inpatient and outpatient practitioners to provide seamless and affordable care with shortened windows for follow up. Participates in code strokes, interdisciplinary rounds, department meetings, active research studies and DNV surveys. Performs chart audits, two day follow up calls, 90 day mRS and ECOG scoring. Facilitates the stroke and aphasia support groups, community engagement, fund raising, and maintains interdisciplinary partnerships.
Staffing system wide to meet demands of various acuity needs including the Emergency Room, Intensive Care Units, Behavioral Health, Acute Rehabilitation Hospital, Medical and Surgical Floors. The expectation is to be a self motivated, timely professional that is flexible and knowledgeable. Able to support diverse areas, possessing ability to quickly assimilate to an environment working safely and confidently per protocols of each unit.
Specialized duties on this floor include direct patient care for chronically ill patients beginning or currently on hemo or peritoneal dialysis. Interdisciplinary collaboration to provide realistic goals and ultimate quality of life. Education with patients and care givers to improve compliance and understanding of medications, infection prevention, and dietary restrictions.
An acute medical environment with adult population varying from drug and alcohol withdrawal to frail geriatric patient. Collaboration with Certified Nurse Gerontologists to develop fall precautions and injury prevention with limited use of restraints.
