Responsible and accountable for direct and indirect patient care across the care continuum, with an emphasis on outpatient care, for designated populations. The position is expected to coordinate services and provide targetedd consultations, screenings, and support services for patients. The Navigator coordinates a triaged plan of care consisting of specific intervntions and/or services centered on meeting the expressed needs of the patien/family within the contexxt of their identified cultural heritage.

  • Coordinates and facilitates initial consultation and screenings in order to assess service needs.
  • Develops initial and updated plans of care to reflect evolving patient needs
  • Is part of a multidisciplinary team which meets regularly and contributes to the development of a comprehensive plan of care based on the patient’s needs.
  • Accurately documents all navigator proceedings through appropriate recordkeeping in the electronic health records system (RPMS/EHR).
  • Develops and maintains cooperative referral relationships; facilitates and tracks referrals.
  • Collaborates and maintains positive working relationships with all services and programs designated by patient’s plan of care to ensure appropriate care.
