• Provide education, support, and counseling for those living with neurodegenerative diseases and their care partners
• Facilitate connection to resources and support within the hospital system and the larger community. Educate patients and families on existing resources and help and manage their care
• Facilitate a support group for those living with dementia

Provide visits to older adults living in their homes or assisted living facilities.

Assist in determining their psychosocial needs and provide case management to ensure they are able to remain in the community safely.

Conduct psychosocial assessments and implement a care plan that is unique to needs of patient and family

Identify patients through the hospital's medical record that are part of a 90 day Medicare program designed to better coordinate healthcare

Conduct psychosocial assessment to the risk level of a patient and plan for their needs upon discharge
