Manage all care transitions through collaboration with the enrollee, community provider staff, ICT, and hospital staff to ensure a safe discharge plan, and a well-coordinated implementation of the plan.

Mobility Assistance for Disabled Patients: Helped patients with mobility challenges by providing support during transfers (e.g., from bed to chair), and assisted with walking or using mobility aids, such as wheelchairs or walkers, to ensure their safety and comfort.
Meal Preparation and Feeding Assistance: Prepared nutritious meals tailored to each patient's dietary needs, including special diets for those with disabilities.
I also assisted with feeding patients who had difficulty eating independently.

Reported these to healthcare providers and families to ensure the appropriate level of care.
Assistance with Therapy Exercises: Helped disabled patients with prescribed physical therapy exercises and other mobility exercises, following guidelines from healthcare providers to improve or maintain their physical strength and range of motion.
Adaptive Assistance for Disabilities: Provided customized care based on the specific needs of patients with disabilities, which included assisting with specialized equipment, helping patients with activities that required adaptive devices, and ensuring their environment was adjusted for maximum accessibility.
