- I was given a list of 20-25 patients a daily whom I would visit, discuss, and fill out their Advance Directive.
- I would visit patients in all units of the hospital and communicate with Nurses and Physicians to plan what is best for the patient's end-of-life care.
- I shadowed physicians in the ICU and Emergency Department.
- Took the initiative to refine the Advance Directive with the help of Salem Health's case managers to ensure high-quality information and provide clarity for patients to gain more understanding.
- Worked with Salem Health's interpreter services to create more accessibility on the Advance Directive for patients who speak a language other than English.
- Ensured Advanced Directives were in stock in all units of Salem Hospital, daily.
- Used excellent verbal skills to engage patients in conversation about the Advance Directive and effectively determine the patient's needs for their end-of-life care stage.
