Resident Assessments:

• Conduct initial and ongoing assessments to understand the social, emotional, and psychological needs of residents.
• Collaborate with other healthcare professionals to develop and update care plans based on individual assessments.
• Work closely with residents, families, and interdisciplinary teams (nurses, doctors, therapists) to create personalized care plans that address each resident's social and emotional needs.
  Ensure that care plans are adjusted as necessary to reflect any changes in residents' needs.
• Advocate for residents' rights and well-being, ensuring that they are treated with dignity and respect.
  Offer support to residents and families through the transition into long-term care, and throughout their stay.
• Provide counseling and support in times of crisis, such as dealing with the loss of a loved one, adjustment to facility life, or any emotional distress experienced by residents or their families.
  Offer conflict resolution between residents or between residents and staff members when necessary.
• Connect residents with external services and community resources such as legal aid, social services, financial assistance, and support groups.
  Ensure that residents have access to appropriate community-based services, both inside and outside the nursing home.
• Maintain accurate and confidential records of resident assessments, interventions, progress, and outcomes.
  Document any changes in the resident’s social or emotional well-being and update care plans as needed.
• Ensure compliance with state and federal regulations regarding social services in nursing homes.
  Participate in audits and inspections related to social services and ensure the facility is meeting all legal and ethical standards.
  

• Plan, prepare and deliver lesson plans and instructional materials that facilitate active learning
• Instruct and monitor students in the use of learning materials and equipment
