Responsibilities included the diagnoses, care and treatment of both acute and chronic conditions. Additionally, I was responsible for managing and building a newly added PACT (Patient Aligned Care Team) to meet the needs of the community. Core duties included obtaining patient histories, performing physical exams, formulating a working diagnosis and developing/implementing a treatment plan according to clinical guidelines; determining appropriate hospitalization, and /or home health care needs. Collaborated with social services and mental health specialists routinely to provide comprehensive health care. Responsible for building a newly formed patient panel, the majority of patient encounters were newly vested veteran's and the visits were comprehensive - 60 minute appointments. This assignment was concurrent with the COVID-19 crisis. Restrictions related to face to face health visits were implemented during the course of this assignment.

The visits at that time were converted to telephone or video visits. Prior to this assignment, I had very limited experience in telemedicine. I now feel competent in this area. I feel especially satisfied that I was instrumental in the effort to provide and maintain access to healthcare for our nations veteran's.
