Punctually arriving to members place of residence, skilled facilities, hospitals or mobile clinics to conduct an assessment (adult, geriatric or pediatric) via Telehealth or in-person that can last up to an hour.

Review and observe a member's current health, medical history, vaccinations, diagnosis

/treatment and medication adherence.

Provide physician referrals, case management referrals, follow-up and clear documentation according to company's guideline and protocol.

Ability to communicate effectively in both oral and written form, handle difficult and stressful

situations with professional composure.

Observe members social environment, welfare check and other safety risks. Perform simple screenings such as PAD, BONE DENSITY, DEE, KED, AIC, GFR, SPIROMETRY, FOBT, venipuncture, and/or obtain stool/urine sample for analysis.

Driving for a considerable amount of time from member to member (15-30mins)

Perform physical exams, recording patient medical history, creating care plans and administering diagnostic test.

Diagnose and treat acute, episodic, or chronic illness independently. Assess and recognize serious conditions and direct patient to ER for advance care when necessary.

Provide a wide variety of clinical services including treatment of acute episodic conditions

( sore throat, colds, flu-like symptoms, lacerations, minor injuries, suture, splint and casting.)

Vaccinations, order diagnostics and laboratory testing, administer or prescribe treatments.

Consult with appropriate medical specialist when necessary; referring patients to specialist within network.
