I remotely supervised a team of visiting nurses and community social support workers who visited people in their homes.

I interviewed prospective employees, provided training, held one to one sessions with each staff member on a monthly basis, and led staff meetings.

I wrote SOPs for staff reference. I audited documentation to ensure compliance with CMS standards. I also carried a caseload that I met with telephonically at least monthly.

• In this position, I provided RNCM services to the dual members (Medicare and Medicaid) in northern AZ
• This included member and family education on their conditions, finding needed resources, communicating with providers and with members insurance company, assisting with referrals and appointments, coordinating needed services, proper documentation, and attending interdisciplinary case conferences. I left this position when an opportunity for a management position became available.

• Was responsible for the oversight, direction, and coordination of clinical care to Hospice of Laramie patients carried out with the inclusion of providers, social workers, bereavement counselor, patients, and families
• Duties include streamlining the referral and admission process, verifying insurances, and developing plans of care
• Was responsible for managing a clinical staff (interviewing, training, supervising, evaluating, and disciplining)
• Audited records for accuracy and addressed corrective actions when necessary
• Enhanced interdisciplinary collaboration through effective communication and coordination among healthcare team members.
• Mentored new clinical staff members for proper onboarding and orientation processes.
• Ensured compliance with regulatory standards and best practices through diligent oversight of clinical procedures, documentation, and staff training.
• Established and maintained effective communication with staff, physicians and community organizations to promote high quality patient care.
• Established a relationship with a pharmacy that provided medication delivery. This streamlined the process for having medications available for patients in a timely manner.

• I was able to continue advocating for Medicaid clients in this role
• I reviewed critical incidents and deaths among the Medicaid waiver participants
• This involved reviewing medical records, monthly evaluation notes, POCs, etc
• I then prepared and submitted a report to the State of Wyoming's 1915c committee along with recommendations
• The goal of this specialty review type was to ensure these participants were receiving adequate services through the Medicaid waiver programs

• I provided case management for clients with Medicaid in southeast Wyoming
• This involved using critical thinking skills as well as coordinating with other agencies within the communities I served
• The goal of my role was to affect positive change in one or more behaviors exhibited by these clients which served to improve health in small ways which, in turn, could reduce Medicaid dollars spent
• I provided education in the areas of targeted conditions such as diabetes, asthma, COPD, etc, as well as a variety of topics with complex clients and medication teaching will all clients who required it
• I worked closely with behavioral health advocates and community health workers
• I consistently had the most billable cases outreached each month throughout the state according to my regional manager
