• Conduct comprehensive evaluations of patients' medical history, current condition, and support systems to determine appropriate discharge recommendations.
• Coordinate and schedule necessary medical tests, appointments, and follow-up care for patients post-discharge.
• Educate patients and their families about post-discharge care instructions, medication management, and available community resources.
• Advocate for patients' needs and preferences during the discharge planning process, addressing any barriers or concerns that may impact a successful transition from the healthcare facility to the community.
• Collaborate with insurance companies, Medicare/Medicaid, and other funding sources to ensure coverage for necessary post-discharge services and resources.
• Maintain accurate and up-to-date documentation of discharge plans, patient progress, and communication with healthcare providers and patients' families.
• Conduct ongoing evaluations of discharge plans to identify areas for improvement and ensure continuity of care.
• Provide emotional support and counseling to patients and their families during the discharge process.
