Day shift LVN.

• Worked with multidisciplinary team to carry out successful treatment plans for diverse acute and chronic conditions. Assisted RN with Regular and PRN visits. Educate family, staff and patients on medications, pain/symptom control. Wound care treatments and fall visits.
• Triage after hours nurse. Answer phones and triage calls to educate family or facilities on medication and symptom control. Send out nurse to evaluate patient. Call in medication refills, called DR. for new orders and comfort calls for patients.

• Provided continuous care at bedside for symptom control and medication management.
• Called Doctor for patient changes and medication changes and collaborated with RN. Called into pharmacy for medication and delivery.
• Educated family, patients and facility staff on disease process, pain/discomfort and end of life symptoms.
• Provided death visit.

Started as a CNA then became an LVN. Provided care for patients in multiple settings as a private duty nurse. Provided bedside care, medication management and wound care. Collaborated with family, Medical professionals and Agency as required to give excellent care for each patient.
