Anemia: Reviewed lab results, Epogen and Iron dosage per protocol and collaborated with MD for protocol dosage discrepancy.

Access: Monitored new patients for permanent accesses. Provided education on teammates how to assess permanent accesses for infection, aneurysm/pseudoaneurysm, good and poor blood flow like stenosis, unhealing wound/scab. Reported to MD any access abnormalities and assisted on making appointments with patients' Vascular Surgeons.

Albumin: Monitored monthly lab outcomes and collaborated with MD and dietitian for Oral Nutrition Supplement needs.

Adequacy: Monitored monthly kt/v, and collaborated with MD on patients who needed increase in treatment time, needle size; checked for any clotting issues; access needs to be evaluated by access center for problems like stenosis; encouraged patient's adherence to dialysis prescription.

MBD Mineral Bone Disease: Collaborated with MD to assess need for Vitamin D adjustments, sensipar administration, or parathyroidectomy considerations.

Fluidwise: Constantly reminded RNs and PCTs to be cautious on patient's weight to make sure it was taken correctly, monitored patients tolerating EDW. Guided teammates how to monitor weight and blood pressure trending. Collaborated with MD the need for EDW reevaluation and/or extra day treatment to avoid hospitalization.

Foot Checks: Ensured timely entry of the Step Ahead Manager's monthly footchecks.

Med Matters: Monitored due dates for patient medication reviews by primary nurses and verified compliance.

Infection: Oversaw Infection Control Manager entered the patient's vaccination and ppd on time. Reviewed SGPT were drawn monthly and Hep C semi annually. Checked that an RCA was done on positive blood culture including notes and AORs. Checked that Infection Control Audits were done monthly.

FHR (Facility Health Meeting): Prepared Facility Health Report or QA meeting, attended the meeting, collaborated with FA, MD, SW and RD to optimize health care strategies.

Assessment, POC and Progress Notes: Monitored scheduled POC meeting was done on time, generated POC on unstable patients (frequent hospitalizations, malnutrition, psychosocial issues).
