• - Conduct daily outbound calls to clients in the CareRing program
  - Perform depression, social, and environmental screenings for each client upon intake and throughout their time in the program

  - Offer additional support and community connections to help clients remain independent and living a connected and safe life

- Process placements for foster homes and complete placement paperwork
- Develop and monitor treatment plans for children in assigned foster home

- Communicate and disseminate information to internal and external support systems
- Arrange and conduct home visits to supervise and support foster parents
