Reported any changes in patient's condition to wife and case manager. I was with patient when he was admitted into hospice and made sure he was comfortable during his terminal transition. Patient passed in October of 2022.

• Provided in-home health care for dementia patient. Duties included medication reminders, shower assist, ADL's assistance. Light house keeping, food preparation. Shopping, laundry and taking patient to and from doctors appointments. Maintained quality care life and dignity will providing private care to Mr. Harvey. Duties also included updating patient records on condition and progress. Assistance with medical equipment as needed. Monitoring vital signs and blood sugar levels.Maintaining a safe and healthy environment and providing emotional support. Patient moved into a facility by family choice.

• Providing private non-medical personal care to a private patient with TBI. Case care management while Assisting patient with daily activities including PT and OT programs as per doctor's orders. Ambulating, companionship, safety and supervision, independent support. Assisting with ADL's. Transportation to and from the facility. Medication reminders. Monitoring and reporting any changes to supervisor and or staff. September 2017 services are no longer need. Patient placed in Hospice.

Monitoring patient's condition, taking vitals, medication reminders. Light housekeeping, documentation, maintaining patient flow charts and or logs. Reporting directly to the case manager and or RN on duty. Maintaining a professional attitude while providing professional care to clients.

Duties included personal hygiene, bathing, peri-care, oral care, cleaning, changing, grooming while maintaining patients dignity. Transferring and turning. Operation of Hoyer lift, gait belt, hospital bed and other medical equipment. Taking vitals, monitoring, collecting specimen samples, documenting and recording changes in patient's condition. Reporting directly to the case manager and or on -call nurse. Providing comfort to family and patients while maintaining a professional attitude. Attending IDG meetings at the office and working directly with doctors and health care professionals.
