Colorectal Cancer Screening, Workgroup team member to update current screening recommendations within EPIC, Working on the team. Helping to compile data, and look at current rates of CRC screening within the system. Created education for providers and patients about CRC screening. Transitional Care Management, Workgroup to improve our TCM visits, Working with the Lincoln Market to pilot a virtual RN to improve TCM visits. Created an ROI and data compilation to show the need and potential financial impact of the virtual RN role.

Advance Directives, Division workgroup to update Advance Directives, Looking at current policy to update for the Midwest division. Also will be looking at how this topic is addressed within the system at various levels of service. Update education that will go to clinic staff/providers along with what goes out to patients
