Emergency shelter Case Manager collaborates directly with personnel, families and children in identifying, developing, and implementing intervention strategies to address behavioral, academic, health, economic, attendance, issues specific to immigrants children.

Ensure UC is provided with Change of Address, Change of Venue form and provide instructions on how to complete and submit forms to the appropriate entities; ensure UC is provided with appropriate phone numbers for follow-up.

The Case Manager will provide disaster case management services to individuals and families impacted by disasters. The Case Manager will be responsible, under the supervision of the Case Management Supervisor, for completing needs assessments, developing case plans and making appropriate referrals for additional or ongoing services. The Case Manager will maintain appropriate contact with the client and ensure quality services are provided and needs addressed in a timely manner.
Case Manager is responsible for providing respond to disasters across the United States, working to connect families impacted by disasters. Immediate Disaster Case Management is a rapid disaster response program that provides operational support, technical assistance, outreach, initial triage and Disaster Case Management Services (DCM) to disaster survivors. In addition, the IDCM Team supports State, local and non-profit disaster case management capacity by augmenting and building capacity where none exist.

· Review and sign off on client records in accordance with licensing, Commission on Accreditation of Rehabilitation Facilities (CARF), and contract standards
