- Keep service delivery plans up to date, including six-month and annual reviews, as required.
- Assist individuals with disabilities in finding and maintaining competitive employment that matches their skills and interests.
- Provide direct support and guidance to individuals at their work sites to help them learn and improve their job tasks and skills.
- Facilitate and promote the inclusion and participation of individuals with disabilities in the workplace and the community.
- Develop and maintain positive relationships with individuals, their families, employers, and other service providers.
- Advocate for the rights and needs of individuals with disabilities, and ensure their safety and well-being.
- Monitor and document the progress and outcomes of individuals, and report any issues or concerns.
- Provide training and consultation to employers on how to accommodate and support individuals with disabilities.
- Identify and facilitate the necessary support for individuals with disabilities to become successful employees, such as assistive technology, transportation, or natural supports.
- Solicit support from the client's family, guardian, teachers, friends, mentors, and others who can assist with achieving the client's goal.
• Provide direct service to clients in line with approved Service Delivery Plans.
- Develop naturally supportive environments to allow fading from job sites, as applicable.
- May overlap with job developers at times to provide identified tasks, as needed.
• Maintain competency in the use of RIGHT RESPONSE prevention, de-escalation, and therapeutic physical intervention techniques.
- Maintain all required areas of SETWorks per the most updated User Guide, and other supplemental materials and requirements.
• Communicate as needed with DDA, DVR, Amerigroup, or counties regarding job seekers.
• Troubleshoots barriers to successful contract and client outcomes.
