Evaluate and treat wound care patients and which involves the use
of various techniues such as application of wound vac, compression
therapy, enhancing circulation, pain management, educate patient on
the importance of pressure relief.
Evaluated and treated a wide range of neuromuscular disease such
as status post CVA, TIA, MS, PD, Neuropathy; Joint conditions such as
OA,RA, gout, osteoporosis;. Musculoskeletal/Orthopedic diseases
such as Acute and Chronic joint sprains, hip fractures (ORIF) hip
replacements, bilateral knee replacements, bilateral above and
below knee amputations. Basic cardiovascular and metabolic
diseases: DM, HTN PVD, CHF, chronic lymphedema, obesity
Established open lines of communication with RN, MD, DO, PA during
the initial phases of evaluation.
Upon discharge, updated each patient’s current health status, prior
level of function and functional gains towards independence
Prescribed and readjusted plan of care for each patient, documenting
and informing changes to PTA, OT, COTA and aides.
Evaluates patients for adaptive equipment needs, obtains physician
orders and coordinates with DME medical supplier to request
equipment such as power wheelchairs, manual wheelchairs, and
walker for qualified patient
Supervises 6 PTAs
Develops and implements plans of care appropriate to the assessment findings and plans for discharge based on patient’s needs
Teaches the patient/family regarding active and passive exercises and patient’s total PT program
Evaluates patients for adaptive equipment needs, obtains physician orders and coordinates with DME medical supplier to request equipment such as power wheelchairs, manual wheelchairs, and
walker for qualified patients.
Makes ongoing therapy assessments, evaluates patient progress, updates the plan of care based on finding, and reports significant changes in the patient’s condition to the physician, appropriate team members and Clinical Manager
