Following job duties are performed

Resource Nurse

• Reviews lab results, imaging Results timely. Conducts thorough patient assessments based on results and triaged accordingly for abnormal results. Education is provided as well. Providers are Consulted as well if needs to regarding lab results.
• STD, UTI, BV, etc Treatments are provided to Patients By following resource nurse Policy of Contra Costa Health Services. Education is provided to Patients on prevention of communicable diseases.
• Follows Providers Orders and Patients are Notified Timely via telephone calls to start treatments and to follow plan of care.
• Patient Educator:
• Teaching is provided to Patients who needs Education On how to manage Diabetes, Hypertension, Obesity, Cholesterol levels, etc.
• Patients are taught, how to Monitor Blood Glucose, using Glucometers.
• Continuous Glucose Monitors (Freestyle Libre and Dexcom) are placed on patients and Demonstrates to patients, how to Self Place them and change sensors.
• Patients are taught on how to safely self Administer Injections, like Insulin's, Ozempic, Wegovy, Trulicity, etc. Return demonstrations are done by patients.
• Medication Reconciliation done. Goes over on Medication list.
• Resources are provided to Patients From Krames website to watch videos and Reading, how to manage certain Health conditions with diet and exercise.
• Treatment Nurse:
• Treatments are provided to patients including Pediatric and adult Vaccine, Injections ( B12, Toradol, Testosterone, Depo- Provera, etc) STD treatment (injectables/Oral), Rhogham administrations, HIV treatments.
• EKG is done as per provider Orders. Point of care testing is done including hemoglobin, A1C, Urine Pregnancy test, Urine Dipsticks, Rapid Strep A, etc. Foley Catheters are changed. Ear Lavage is done as per Providers orders.
• Establish Care :
• Pre Screening is done for Patients who are new to Contra Costa Health Services or have not seen More than 18 months. Medication Reconciliation is done, Medical History, Social history, Family History is updated. Necessary Labs are ordered before PCP appointments. Social worker or Health Navigators Referral are placed, if Patients needs any assistance.
• Clinic Nurse:
• When in needs works in clinic with Provider. Patient Intake and discharge is done. After visit summary is discussed with Patient in detail. Any new orders, Labs, Medications, imaging, Referrals are informed to patients and Education provided on how to follow those orders. Patient Understandings are confirmed via verbal demonstration instructions back.
• Team Leader:
• Works with Team, how to handle in basket messages within timeframe. Assignments are given to Team members (LVN and CMA's) based on priority and scope of practice. My chart Advice messages are handled timely and patients are advised accordingly based on symptoms or needs. Goal of forwarding messages to providers that is about 10% is met.
• Enhance patient satisfaction by providing compassionate, holistic nursing care that addressed physical, emotional, and spiritual needs.
• Quick Response Nurse:
• Quick Response is provided in emergent situations and patients are triaged based on symptoms.
• Charge Nurse:
• Works as Charge Nurse, when clinic Charge nurse is out on Leave or Vacation. Clinic Situations are handled and managed, Coverage and Breaks are provided. Medication orders are placed etc and follows Charge nurse duties.

• Works with Providers in the clinic, follows providers instructions to provide patient care by prioritizing tasks. Follows all the rules and policies, maintain patients safety all the time, Practice good Communication skills. Works with team and independently to provide good customer service and care to patients. Ensures patients Privacy.
• Works as a treatment nurse, Administers pediatric and adult vaccines. Administers injections as per providers orders as well, for example, B12 Injection, testosterone, Depo, Makena, Vivitrol, etc,
• Works on Patients' messages, refills requests, lab results, prior Authorization, by following standard turn around times. Answers patients phone calls with good communication skills. Deals with pharmacies to make sure patients get their prescription on time without any delay.
• Performs Point of care tests, EKG's.

• Directly works with patients by following standards of practice and regulations. Administers medications, insulin, Monitoring Blood Glucose, Admits patients by performing full head to toe assessments, entering all hospital discharge orders in the Electronic Records.
• Time Management always been followed. Wound treatments including wound Vacs, Dressing changes. Catheter insertions etc performed as per providers orders.
• Change of Conditions reported immediately and assessments done accordingly following facility protocols.
• Hand off report provided to next shift thoroughly.
