Blessed to have the opportunity to care for my 19 year old medically complex baby now adult. I am excited to apply for the Hospice Liaison position at JOL Healthcare, Inc. With a background in healthcare, patient education, and care coordination, I am passionate about providing compassionate support to patients and families navigating care they deserve. My life experience in transplant coordination both heart and kidney and patient advocacy has strengthened my ability to communicate effectively with multidisciplinary health care teams. I have excelled in building relationships with healthcare providers, understanding many treatment options, and facilitating seamless transitions. I have had the honor of caring for my medically complex infant of 3 months old to now a young man. His life and mine have been a non stop medical journey of a lifetime. Due to the miracle of organ donation he is alive today, and a special living donor, he is here now. My ability to manage 31 medical providers, 12 meds 2 x a day, coordinating care of transplant teams, oncology, infectious disease, ENT, billing for insurance approvals, overrides for necessary medications, numerous calls to coordinate emergency care situations, follow up appointments while living away from our main hospital for care.
