  • Monogram has a contract with Cigna Insurance, and it was my job as a licensed care manager, to help the Cigna patients of whom have kidney disease, work towards remaining hospital free. This was done by creating both long-term and short-term goals.
  • I drove to each patient's home in both NJ and PA to educate them on the following: local resources- such as behavioral health referrals, kidney transplant referrals, housing resources, in home health care, palliative care, at times APS referrals and general kidney health knowledge such as controlling blood pressure and eating less sodium.
  • Utilized My Directives software to create advanced care plans with patients of whom desired to do so.
  • I created a virtual Northeastern Regional Social Work group which helped gain increased knowledge of local resources and answers to questions regarding difficult cases. This group met 1x/month.
  • I participated in weekly virtual multidisciplinary team meetings to include: our region's medical director, all supervisors, social workers, and nurses. We discussed all escalated cases- which included patients that had extremely low kidney function and were in fear of hospitalization. We also discussed patients with multitudes of SDoHs as homeless patients, APS cases, etc. The purpose being to create and adjust each escalated patient's current long and short-term goals.

Creating genograms to accurately notate supports or lack thereof 2. Evaluating each candidate's psychosocial demeanor 3. Evaluating their drug, alcohol, tobacco history- to list a few.
  • Presented each patient's psychosocial assessment at our weekly multidisciplinary meetings. The following individuals are present: all transplant surgeons, our 2 nephrologists, all transplant coordinators (RN's), a dietitian, and I. During these meetings each patient's status is created-active on the list, inactive etc., and, a game plan is put into place for each patient enabling them to work towards getting on the transplant list.
  • Helped patients with insurance questions regarding: Medicare, Medicaid and EGHPs in order for them to qualify for transplantation.
  • Lead a Pre-Transplantation Support Group for patients and their family members. Some of the patient created goals included gaining support and knowledge to ease their anxiety and depression while waiting for transplantation.
