Responsibilities include performing treatment plan and monitoring progress for wide range of conditions including nerve, muscle, and joint dysfunctions.

• Daily interaction with multi-disciplinary team including Doctors, NP’s, Nursing, Social Services, Administrators and Other discipline for early and successful rehab

• Conducts weekly team meeting with therapy staff for discussing current plan of treatment, recommending treatment approaches for meeting patient goals and staff education.

• Conducts interdisciplinary care plan meeting with the immediate family/caregiver to discuss patients progress and directs appropriate plan for successful rehab and recovery.

• Managing Ventilator and Trach dependent patients with diagnosis including COVID-19, acute and chronic respiratory failure, CHF, Parkinson’s, CVA, MS., etc.

• Documentation of daily notes, weekly progress notes, updated plan of care and discharge summary for the patients using computerized software: Optima and Casamba

• Responsible for establishing home exercise program and functional maintenance program for the patients to maintain functional mobility.

• Planning and administrating Therapeutic exercise, Gait training, Neuro-Muscular re-education, Therapeutic Modalities etc. to prevent and treat several dysfunctions.

• Experienced in managing a high volume case loads.
