  • Responsible for providing case service coordination up to 21 participants diagnosed with an intellectual disability and may have co-occurring disorders, complex medical issues, and behavioral needs.
  • Perform initial intakes and routine assessments to determine client’s interests, strengths, needs and evaluate progress.
  • Develop service plans with the client, family, caretakers, or other relevant parties; that are clear, measurable, and person centered.
  • Conduct home visits, office visits, community site visits to collect information from residents. Identify gaps in services, advocate on behalf of clients, coordinate and monitor service quality.
  • Utilize knowledge of the local community to assist clients to link and navigate through a variety of social service organizations and public agencies.
  • Performs a range of administrative quarterly and annual deadlines, such as managing service plans, documentation of clinical records of services, monitoring budgets, correspondence, and following any other procedures and policies as required to meet state licensing and federal regulations.
