I have extensive experience with my nursing background in duties in providing excellent patient care by coordinating services with physicians, nursing homes, assisted living facilities, home care providers, specialized services such as hospice and ancillary services, provide social service assistance with family counseling, complete advanced directives, insurance/financial assistance, legal issues such as court commitments and utilization review looking at appropriate level of care comparing diagnosis with intensity of services and providing insurance companies with medical information with inpatient and skilled care admissions. While working for United Healthcare I worked from home and had a case load of members and responsibilities were to provide health education, coordinate services with medical providers/specialists, perform assessments according to diagnosis and need, document all aspects of care related to case including medications and care planning with emphasis on high care needs.
