• Assist patients with their ADLs and ensure comfort and dignity while doing so
• collaborate with RNs and supporting them when needed
• monitor vital signs and report any abnormal findings to the RN
• answer call lights promptly and providing safe care by not leaving the patients side
• frequently repositioning patients who are immobile or who may need extra help to prevent pressure injuries/ulcers
• document vitals, ADLs that were done, intake and outputs in a timely manner and before shift is over
• educate patients on the importance of calling for assistance when getting up
• empty drains, foley catheters, and colostomy bags
• regularly round on my patients at least every 2 hours or more if able to
• provide therapeutic communication with the patient to build rapport and enhance a safer patient environment

I worked night shift. I provided brief changes to the patients who were incontinent and notified the nurse if there were any changes or concerns. I also provided vital signs to the nurse when needed. Answered call lights and provided patient comfort and safety by adjusting bed rails and equipment. I offered food to patients who got hungry throughout the night including feeding the patients. I also made sure to determine whether or not the patient was NPO or on fluid restrictions. I also documented patient care information accurately in electronic health records systems.
