Manage caseloads of 20-25 patients weekly and provide

palliative/hospice care in home and facility settings ranging from

assessment to development and implementation of care plans to direct hands on care including vitals, medication administration including oral and IV therapy, provide wound care and monitoring, coordination of care of patients services, ordering medical equipment and patient supplies/medications, communication/updates to physicians, families and facilities, complete a multitude of forms/charting as to stay in compliance w/state/company guidelines including medicare guidelines, multi task to complete tasks in an efficient and timely manner, maintain independent work schedule w/flexibility to cover unexpected needs of patients and families. Inform patients/families and facility staff of medicare coverage for medications, benefits and therapy's.

Implemented new floor assignments based on evaluation of staffing requirements, acted as patient advocate and implemented total patient care through a team nursing process covering 8-10 patients per shift, utilized strong assessment skills to determine necessary patient care, assessed patients in active withdrawal and provided interventions to manage physical and psychological withdrawal symptoms, recorded patients' medical history, vital statistics and test results in medical records, administered and monitored IV therapy and hourly oral medications for medical/surgical/stroke/cardiac unit. Multi tasked efficiently to provide excellent care to all patients every shift while completing charting and updating patients, families and physicians and staff.
