This is a moonlighting position where I provide internal medicine coverage for patients undergoing inpatient rehabilitation (IPR).

I work to prevent patients getting more sick and needing to be transferred from the IPR to the Emergency Department for more acute care.

• Services is provided at Trihealth Rehabilitation Center, Cincinnati, OH and Good Samaritan Hospital, Cincinnati, OH.
• For the Trihealth Rehabilitation Center, I provide medical services with a goal to reduce patient been sent out from the Inpatient Rehabilitation (IPR) back to the Emergency Department.
• For Good Samaritan Hospital the work will be at the psychiatry unit; and it will be providing hospitalist/internal medicine services to inpatient psychiatric patients.
• I am not a direct employee of K&G Dedicated Long-term Care Associates, LLC.
• Services is a subcontract from K&G Dedicated Long-term Care Associates, LLC (K&G DLCA) to GLOJOE LLC, which is my company.

Corporate location is at Canton, Location.

However, I work/provide service at Mercy Hospital- West in Cincinnati, OH.

Nortunist handling admissions; procedures and cross covers (inpatient medical care issues).

Proficient in intubation; vent management; central line placement; BLS and ACLS.

I am an employee of US Acute Care Solutions.

I provide Hospitalist and Nocturnist services . I handle admissions; procedures and cross covers (inpatient medical care issues). Coordinate with nurses; case management; family and patients on discharge planning; medical and social issues.

Proficient in intubation; vent management; central line placement; BLS and ACLS.

I initially worked as needed (prn) Knox from 10/22 to 9/23 where I was providing services as an independent contract via my company, GLOJOE LLC. The hospitalist group at that time was run by many independent contractors and a few hospital Hospitalist employees.

In September 2023 the Hospital gave the hospitalist service to Vituity in a contract. Through my company GLOJOE, I partnered with Vituity and provided full time Hospitalist/ Nocturnist services; later I went to a prn (as needed) position.

This is my own company via which I provide Independent Hospitalist/Nocturnist Services.

Via GLOJOE LLC, I provide Hospitalist service directly or through other companies/agencies.

From 10/23 to present, I am working via USACS to provide nocturnist service at Mercy Hospital West-Cincinnati, OH.

I am an employee of USACS and not an employee of Mercy Health System (Mercy Hospital)

It is as needed (prn)moonlighting position through Echo Locum Tenens (a subsidiary of Sound physicians). Initially I worked as an individual but later on switched my work classification to GLOJOE LLC.

I am not an employee of Cleveland Clinic - Union Hospital.

I am also credentialed at Selby Hospital which is part of the Marietta Memorial Health System but I have never actually worked there (Selby Hospital)

Via Echo Locum Tenens (subsidiary of Sound Physicians) I have had the opportunity to work at many hospitals. These opportunities included my first encounter with Mercy Health System in Cincinnati, OH (10/2020-06/21); Cleveland Clinic -Union Hospital (01/20-present); Mercy Medical Center, Canton, Ohio, (02/20-12/31/21); and Mount Carmel Health System

(08/18 -3/19).

I worked here as a Nocturnist primarily via Bloomington Medical Services which is a subsidiary of Wooster Community Hospital. I am not a direct employee of Wooster Community Hospital.

I initially as a full time employee with Bloomington. In September 2023 I switched to an independent PRN (as needed) physician position through GLOJOE LLC; and still via Bloomington but I am no longer a direct employee of Bloomington Medical services.

Location of job is at Wooster Community Hospital. I worked at Wooster Community Hospital as a Nocturnist primarily via Bloomington Medical Services ( a subsidiary of Wooster Community Hospital). I am not a direct employee of Wooster Community Hospital.

I am(was) not an employee of Good Samaritan Hospital (Trihealth from July 2018 to present). However I was a Resident Physician of Good Samaritan Hospital (Trihealth ) from 06/15 to 06/18.

While credentialed at TriHealth I was also credentialed at Bethesda North Hospital ; and Bethesda Butler, Hamilton, OH. Further, I was also credentialed at McCullough Hyde Hospital, Oxford, Ohio, but I never worked there.
