As well, I have extensive experience working with vulnerable and marginalized populations, such as adults with severe mental health diagnoses and addictions, those experiencing homelessness, adults with developmental disabilities, and children with emotional and intellectual deficits. I have thorough experience providing services to help patients navigate the healthcare and social service system, as well as in educating patients on building the skills to live independently in the community. In addition, I am capable of integrating care plans with both patients and providers in the spirit of collaboration to help the patient attain their best health outcomes.
